Incident Reporting
The Incident Report form is available for download, and there are three options to submit a report.
How to Submit a Report
To report an incident, click the download button below to view and download the form. Complete the form and include any attachments as necessary, then choose one of the following submission methods:

Email Submission
Scan the completed form and all necessary attachments. Email the package to [email protected]

Council Mailbox
Deposit the completed package in the Council mailbox located in Tara Hall.

Hand Delivery
Hand deliver the completed package to a current Sahnoan Council member.
When your Incident Report Form is received, you will receive an email outlining the steps to bring the incident to a close.
